Identify Yourself

One of the ways to insure your safety is to identify yourself. When you are admitted to the John Dempsey Hospital, you will be given an identification bracelet.

• Wear your white I.D. bracelet at all times. If your bracelet comes off, ask for a new one.
• Check that your name and birth date are listed correctly on your I.D. bracelet.
• Have all your health care providers check your I.D. bracelet before any procedure, surgery or test is performed or before medication is administered.
• If you have medication allergies, you will receive a green bracelet in addition to your white I.D. bracelet.
• If you are at risk for fall, you will receive a yellow bracelet to wear in addition to your white I.D. bracelet.
